Reporting Guidelines and Requirements

Narrative and Financial

National Aetna Foundation grantees are responsible for providing narrative and financial reports approximately every six months during the applicable grant period. The resources and information provided below are meant to guide both National grant seekers and recipients through the Aetna Foundation’s reporting expectations and requirements.

Interim Narrative Reports - Interim narrative reports are collected approximately every 6 months, as specified in the terms of grantees' contract with the Aetna Foundation, for the duration of a grant, until the close of the project. National grantees are expected to provide detailed information about the progress of the project/research, explain how the project/research is meeting established goals, and describe what activities are planned to meet these goals.  These reports seek to capture any unexpected challenges, barriers or issues encountered, as well as provide details on what planned activities may not have been completed according to schedule.  Upon review, the assigned Aetna Foundation program officer may contact the grantee for additional information.

Final Narrative Reports - Final narrative reports are designed to collect information on the progress of the entire project/research, including any successes or challenges that accompanied the project.  All summaries of data, copies of abstracts from presentations and/or conference session presentations, communication materials, as well as any other documents or materials appropriate for the Aetna Foundation to make a final evaluation on the outcome of the project, should be included.  Upon review, the assigned Aetna Foundation program officer may contact the grantee for additional information.

NOTE: Any deviations from the planned activities that were presented in your original proposal must be reported and addressed both as they develop and in your narrative reports. Additionally, we ask that you send electronic versions of your products/communication materials as soon as they are completed, and not wait until you submit your narrative reports.

Financial Reports - Formal records of the financial activities related to the grant are required for all Aetna Foundation supported projects.

NOTE: Each National award recipient will receive a customized payment and reporting schedule that includes narrative and financial reporting deadlines.  While sample narrative and financial templates are available for viewing in the box below, all financial reports most be submitted in Excel, and the narrative reports as a Word document.  The assigned Aetna Foundation program officer will send electronic copies of each template based on the payment and reporting schedule and also as companion to the contract.
